Earlier models require a 2 hole cam bearing on # 5 along with machining a flat on the cam journal. Is this required when there is not a full groove in the #5 cam bore ?

The '65 and '66 big blocks had a grooved rear journal and internal groove in the bearing, no groove in the housing bore. Crane Cams says the cam groove is centered to the journal, .187 wide and .109 deep. Use cam bearing set for 65-66.
I thought the small block always had a groove in the housing bore.

There was some very early 265 and 283 blocks that also required a groove cut in the rear journal as well because there was no relief holes in the rear of the cam journal to bypass oil from the cam plug as well.

I am restoring a very old 283 at this time that requires that same journal design using an NOS GM cam

My bad there-guess I forgot. Haven't seen one of those SB for a spell. Now I will probably take in one of those 265 chevys that didn't have an oil filter. Glad you had one in the shop to help with the question.
